Where is Honoka'a? | Where is Hamakua Health Center?
THE HAMAKUA HEALTH CENTER -
has a long history along the Hamakua Coast. Our roots are firmly planted in the very soil of Hawaii, through the sugar cane fields, in the people, in the very breath of our island. Generations of families have chosen Hamakua Health Center for comprehensive health care offered in the spirit of aloha.
CARE FOR ALL PEOPLE OF HAWAII - We believe each person deserves access to outstanding, state-of-the-art health care and highly skilled health care providers. Our doors are open to all people of Hawaii - regardless of income or ability to pay. For those without income or insurance, a Sliding Fee Scale is offered.
A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) - The Hamakua Health Center receives funds from our Federal Government to provide comprehensive, culturally competent, quality primary health care to medically underserved communities and vulnerable populations. We are held to rigorous performance and accountability standards including administrative, clinical and financial operations. We must measure the effectiveness and quality of our services and continuously evolve our programs to achieve the greatest impact on population health.
REDUCING HEALTH DISPARITIES - The Hamakua Health Center participates in the Federal Government’s Health Disparities Collaborative. Often minority populations, women and children, and people with lower incomes do not receive timely, comprehensive care for illness, which can result in higher rates of death from long-term chronic illnesses like heart disease, cancer, stroke and diabetes. To eliminate these disparities, health care organizations must change the way they deliver care. The Health Disparities Collaborative calls for this transformation; we are honored to participate in and assist with this goal.
INTEGRATED HEALTH CARE - Our primary care providers work side-by-side with trained counselors, in an integrated health care model, to detect early onset of emotional stress and common behavioral disorders such as anxiety and depression. Providing appropriate behavioral health treatment in the primary care setting presents an important opportunity to reach people who cannot or will not access care in the specialty mental health and/or substance abuse setting. Treating behavioral health in a primary care setting is crucial, as early detection and treatment can prevent problems from developing into more disabling disorders.
CARING FOR OHANA….CARING FOR YOU -
Hamakua Health Center is a nonprofit organization governed by a community board composed of a majority of health center patients who represent the population we serve. Not only do we provide comprehensive primary health care services, we offer supportive services that include education, support groups, translation services and transportation to and from appointments. Quite simply: We believe in caring for ohana… and caring for you.
